Registration of Business Names
Required Documents
- Certified copy of the trade license by the Grama Niladhari (Must be from the relevant local authority)
- Rs. 5 / = valued stamp should be stamped in relavent application. If it is a joint venture, a stamp of Rs. 50 / = on the back stamped and should be singed by partners infront of a Justice of the Peace
- Copy of the deed of the land on which the business is carried out (Certified by the Grama Niladhari)
- Form obtained within 14 days regarding the deed of the land (should be recommended by the Grama Niladhari as applicable to the place of business)
- If the land is owned by another owner, the will of the owner should be submitted by an affidavit (with Rs. 50 / =) infront of a Justice of the Peace. Copies of the agreements, whether tax or rent based, should be certified by the Grama Niladhari.
- If the place of business is not in the area of residence. Certificate of residency obtained by the Grama Niladhari of the division in which he resides should be co-signed by the Divisional Secretariat.
- A copy of the National Identity Card should be certified by the Grama Niladhari.
- A photo of a nameplate with clear business name, address and phone number.
- Other documents to be submitted depending on the nature of the business
(Ex: I. Nurseries and Seed Marketing - Department of Agriculture Recommendation (Agriculture Instructor)
II. Defense Services - Police Reports, Recommendation of the Ministry of Defense
III. Building Construction Contracts - ICTAD Certificate, Copies of previous Project Study Certificates, Letters Certified by the Grama Niladhari including Machinery and Storage Facilities, Number of Employees))
* If the entrepreneur is a public servant, a copy of the formal approval issued by the Head of the Institution to engage in the business
* License fee is Rs. 250 / =, delay fee is Rs. 80 / = (may Changable)
Obtaining an Identity Card for the first time
Required Documents
- Recently obtained birth certificate and its photocopy
- Receipt obtain from the photo studio for online photos
- If the job designation needs to be included in the identity card, a service certificate should be sumbitted which obtained within 06 months
- If above sixteen years, should submitted පු.ලි.දෙ/මෙ/02/03 application form with Receipt of payment of Rs.250/-
- Rs.45 stamped address unwritten long envelope

Obtaining an Amendment Identity Card
Required Documents
- Recently obtained birth certificate and its photocopy
- Receipt obtain from the photo studio for online photos
- If the job designation needs to be included in the identity card, a service certificate should be sumbitted which obtained within 06 months
- Identity card currently in use
- If the identity card number is unclear, a written proof of the identity card number
- Rs.45 stamped address unwritten long envelope

Obtaining a Lost Identity Card
Required Documents
- Recently obtained birth certificate and its photocopy
- Receipt obtain from the photo studio for online photos
- If the job designation needs to be included in the identity card, a service certificate should be sumbitted which obtained within 06 months
- Police complaint received within 06 months
- Written proof of the identity card number (except the copy of the old identity card) (photocopy of a bank book, photocopy of the driver's license, photocopy of the valid passport) If none of these the letter should submitted by the applicant stating that applicant not have any proofs to certify his/her identity number and letter should be recommended by the Grama Niladhari and signed by the Divisional Secretary.

Preparation of an identity card under normal service
After completing the application form of the identity card by the Grama Niladhari and handing it over to the Identity Card Division of the Divisional Secretariat, it is checked and if there are no any deficiencies, it is singed by the Divisional Secretary and upload in the ID head office computer system (SCAN and EQD). The head office then prints the identity card and sends the letter to the address mentioned in the applicant's application form by registered post.
Preparation of an identity card under one day service
After completing the identity card application form by the Grama Niladhari and handing it over to the Identity Card Division of the Divisional Secretariat and inspecting it, if there are no any deficiencies,it is singed by the Divisional Secretary and upload in the ID head office computer system (SCAN and EQD) and allocate a number to obtain the application form from the one day service and submit the application form to the applicant with the allocate number.Applicant should go to the person's Registration Department in Batharamulla and hand over the application on the due date and time.The head office will issue the identity card to the applicant on the due date and a fee of Rs.1000 will be charged for it.
Activities related to school applications regarding ID
.Application forms and QR stickers required for the preparation of identity cards are issued for the candidates appearing for the GCE (O / L) examination in the schools belonging to the division.When the completed applications from the schools are handed over to the Identity Card Division, their deficiencies are checked and a PIV is prepared and issued to pay for the applications. Then the instructions are given to mention the number of the receipt obtained from the shroff by paying the fee, and to mention the date and signature and to send the application to the head office.
